Fair/Show CVI Checklist for Owners
2025 Fair Season
Call no later than 3 weeks prior to your first show/fair to schedule your exam for your health certificate.
*Calling to schedule less than 3 weeks prior to your first show/fair will make your appointment subject to an Urgent Scheduling Fee of $145 or unable to be scheduled.
Fill out the Livestock/Animal Identification Form and Fair/Show Schedule Form sent to you via email upon your appointment being scheduled. This will be due no later than 3 business days prior to your scheduled appointment. Please email all forms to records@roxburyanimalclinic.com. If we do not receive these forms by the deadline, your appointment will be canceled.
*In order to standardize our process and ensure all animals and shows are captured, we will be requiring these forms to be filled out in their set format as sent to you.

- Fill out all required fields in date order
- Health certificates may include multiple fairs/shows, but separate ones need to be issued based on different species and different states (All New England shows may be on the same health certificate)
- Health certificates, once signed are valid for 30 days and therefore the first show listed should be within 30 days
- Each state may have different requirements for entry. Be aware of these requirements, and if in doubt, the Farm Call Technician you are working with can help you as long as you communicate in advance.
On the day of the appointment, have all livestock/animals caught and organized. Exams are charged by time in 15 min increments at the rate of $280 per hour and not by animal.
Monitor your email for health certificate draft(s) from your Farm Call Technician, review for accuracy, and then approve or communicate changes via email.
*Drafts are not signed by the doctor and therefore are not valid health certificates. The purpose of the draft is for you to confirm accuracy. If you confirm via email that all details are correct, once signed, changes may not be made to the health certificate, and a new certificate and a new exam (depending on timing) may need to be done at your expense.
*All certificates must be signed within 10 days of the exam, or a new exam may be needed, so ensure your response is prompt to allow the certificate to be signed in time.
Note: The purpose of the exam and health certificate is for a qualified veterinarian to attest to the health of each animal and lack of communicable diseases in order to attend a fair/show or to cross state lines. If for any reason, one of your animals does not pass the exam for the health certificate, a new appointment will need to be made, and another exam will need to be done by the doctor, at the owner’s expense once the health concern is resolved in order for a health certificate to be written for that animal.
